Gift of Hearing 2010         

 

Advanced Audiology Concepts is proud to offer the Gift of Hearing 2010 to a deserving individual from our community with hearing loss.  The recipient will be awarded a pair of hearing aids and fitting services.  The hearing aids are valued at $1900.00.  All entrants are eligible for a free hearing screening to assess hearing aid candidacy and hearing aid consultation.  

Contest Rules

1.  To enter send the following information:

   i.       Name of intended recipient 
   ii.      Address of intended recipient
   iii.     Telephone number of intended recipient 
   iv.     Name of a contact person (Can be same as recipient)
   v.     Telephone number of contact person

 2.  Send entries to Advanced Audiology Concepts at: 
          8897 Mentor Ave.
          Mentor, Ohio 44060  3.  All entries must be legible. 

4.  All entries must be received by 12:00 PM October 1, 2010. 

5.  Entrants will be contacted by Advanced Audiology Concepts to schedule a FREE hearing screening and hearing aid consultation.  If entrant requests a complete hearing evaluation it will be scheduled at another time and billed to the entrant's insurance.

6.  Test results and personal information is kept confidential. 

7.  The recipient of the Gift of Hearing may be required to release his or her name to promote the Gift of Hearing program.  8.  There is no right for appeal of the final decision.  The decision is final. 

9.  Employees and immediate family of Advanced Audiology Concepts are not eligible.
